NEW HIRES
If you are getting ready to start your first assignment with BTHR BeneTemps, please download and complete the New Hire Paperwork Package. The package includes:
- Employment Application
- I-9 Form (Employment Eligibility Verification)
- W-4 Federal Tax Withholding Form
- Direct Deposit Form
- Conduct Guidelines
- Employee Handbook Acknowledgement Form
- State Tax Withholding Forms
(complete for the state in which you work – forms are available to download below)

RE-HIRES
If you are an employee who has worked for us before, there are only a few forms you may need to complete. You can print the Rehire Paperwork Package only if you have changes to the information we currently have on file for you. The Rehire Package includes:
- Direct Deposit Form
- W-4 Federal Tax Withholding Form
- State Tax Withholding Forms
(complete for the state in which you work – forms are available to download below)
